Application Form (New Students only)
Below is the application form for St. Leo’s School. Please be aware of the following:
- You will need to create a user account to create an application.
- Your account will remain active for as long as your student is enrolled or in the enrollment process.
- Once your account is created, you will be able to update and/or edit your application.
- Your application account is NOT your Powerschool parent portal.
- There is one application per family, you do not need to fill out a separate form for each child.
All information in the application is confidential and we do not share your information.
