Immunizations and exam records must be submitted to the school nurse before the first day of school. Students can not start school until ALL forms have been completed.
Common Questions about Illness
Please notify the school before 9 am if your child will be absent. You can call the main office directly or you can email the school nurse at [email protected], please be sure to include your homeroom teacher in the email.
An absence of more than 3 days will require a doctor's note to return.
Please return school nurse and/or office emails as soon as possible, we may be calling because your child is sick and needs to go home.
If you child has any of the below symptoms, they can not return to school until they are symptom free (without the use of fever-reducing/symptom relief medicine) for 24 hours.
Students who return prior to the 24 hour time frame will be sent home.
Medications at School
If your child has asthma, please complete the Asthma Treatment Plan document below. Inhaler medication must be sent in the original box with the prescription on the label.
If your child has any food allergies, please complete the Food Allergy Care Plan below. If your child has an epi pen, medication must be sent in the original box with the prescription on the label.
Any medication a child needs to take at school must have a doctor's note, medical administration form and come in the original container.
